Register as a New Patient

To Register at King George Road Surgery you must live within our Practice Catchment Area.

To register with King George Road Surgery:

  • Visit the surgery after 10am – 6pm to collect a paper registration form

You will need to come into the practice to sign the forms and provide evidence of your identity and address (if you have them).

Acceptable forms of Identification

  • Photographic ID: Passport, Drivers Licence etc.
  • Proof of Adress: Bank Statement, Utility Bill or Tenancy Agreement

You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Once you have registered, you may be required to attend a new patient health check so we have all your information as soon as possible.

When you register, it’s helpful to have your NHS number.

Named accountable GP

All our patients have an allocated named GP, which is your registered GP. If you wish to know who this is, please ask at reception.

Please note that you can still choose to see the GP of your choice.
